JOB DESCRIPTION: Duties of the position will consist of but are not limited to: Developing and maintaining long-term contact with clients, client families, and service providers for medical, social, educational, and related service needs. Under the supervision of the Authority Services Program Manager, provides an array of continuity-of-services assistance and support, including targeted case management and rehabilitative services, to persons with mental retardation or a pervasive developmental disorder. Responsible for identifying the nature of the presenting problems and specific services and support needs of individuals; planning, advocating, and coordinating the delivery of needed services, including emergency services.
QUALIFICATIONS: Bachelor’s degree in a field of human/social services from an accredited college or university. Two years direct experience with individuals with intellectual and developmental disabilities preferred. A valid Texas driver’s license and good driving record is required along with a copy of a valid auto insurance card.
PHYSICAL ABILITIES: Must be able to bend, stoop, kneel, crouch, squat, lift 25 pounds, sit for an extended period, and reach overhead.
