1. Beneficiary must be the named person on mortgage, rent, utility, loan, or other bill/invoice presented for financial support.
2. Financial assistance payments are not made to the Beneficiary, but to the vendor.

1. Proof of Veteran Status and 180+ days of service
2. Budget worksheet (Click here)
3. 90 days of bank statements to validate financial need
4. Copy of most current bill due
5. Additional documents may be needed after review
